Attention Big Island And Hello Hawaii P.D.






Good morning!!  I just posted on Craigslist and thought to let you know too!!


Wow, your amazing volcanic activity reminded me to request your island citizens a question. I would love to live on the Big Island as I know that you have horses!! I am in the process of 'Porting' to San Francisco, my hometown, in fact I was born in 1965 and that story is as that story goes however in inasmuch my mother and father decided to call it forward and made the lives of two others marvelous!! Now that I am 53 years old, single and my four kids are grown I believe that living where people are the happiest on earth would be fantastic. The fact that those plate lunches have anything to do with this would only admit that I loved those stands!! Clinical Case Summary; dated 08/09/2018: The injured worker was a 52 year old, who sustained an industrial injury on July 27, 1999. The injured worker was undergoing treatment for radiculopathy of the lumbar region and L4-L5 anterior discectomy, decompression and fusion with instrumentation with an iliac rest bone graph.
According to progress note of May 11, 2017; the injured worker's complaint was low back pain. The pain was rated 10 out of 10 without medication and the average pain was 7 out of 10. The pain was rate 5-6 out of 10 with medication. The injured work no medications at this time. The injured worker was requesting medications for the rear times when the symptoms were intense to deal with utilizing over the counter medications. The injured worker was only taking the over the counter medications at this time. The objective findings were a well-developed and well-nourished injured worker in no apparent distress. The injured worker walked in a mildly stooped fashion. The injured worker had a stooped gait. The lumbar spine examination noted no limitation in range of motion. The heel to toe ambulation was normal. The Babinski's test, straight leg raises and Pace's sings were all negative. The recommendation was a low dose Morphine to use on rare occasions seemed to be sufficient.
The injured worker previously received the following treatments Morphine Sulfate ER 10mg one tablet every 8 hours as needed since April 2017 and lumbar spine MRI on January 17, 2005. The RFA (request for authorization) dated June 19, 2017; the following prescription was requested for Morphine Sulfate ER 10mg#30. The UR (utilization review board) denied certification on June 22, 2017; prescription for Morphine Sulfate ER 10mg #30. Update: Prescription for Morphine Sulfate ER 10mg #30 is still held in pharmacy in The County of Marin awaiting approval from Arrowpoint Capital Worker's Compensation Insurance Company in-order for the Worker's Company to be charged and not requiring me to pay for the prescription and then seek reimbursement, notoriously done however due to the financial restrictions following the loss of income in 2007 I have had to now and currently wait for the Worker's Compensation Companies to pay in-full for every prescription. This process of approval extends opportunity to the Worker's Compensation Company to deny the request. A long and arduous process however this approach from Insurance Company both 'public and private' are not uncommon practices.

Dyer is the City of Fresno’s 26th Mayor, sworn into office on January 5, 2021. Mayor Dyer came into office with a vision of uniting Fresno – seeking an inclusive, prosperous, beautiful city where people take pride in their neighborhoods and community. A government that listens, keeps its promises, and is owned by the people. This is his “One Fresno” vision. Upon assuming office, the Mayor’s Administration successfully implemented “Project Off-Ramp,” an initiative that relocated more than 600 unhoused residents off Fresno’s freeways and into safe housing, with the goal of permanent housing and a productive life. He has the distinction of being the longest-tenured police chief in Fresno’s history. This experience gives him unique insights into the City’s operations and has helped drive a motto that is central to his One Fresno vision – “We Work For You.” This is his compact with residents, a promise that city government’s role is to serve the public. It also ensures his continued work to enhance public safety throughout Fresno. Mayor Dyer was born in Fresno but grew up in the Fowler area and graduated from Fowler High School. He earned a Bachelor of Science degree in Criminology from California State University Fresno and a master’s degree in management from California Polytechnic University at Pomona. Chosen as a Paul Harris Fellow for Rotary International, Mayor Dyer was recognized in 2018 with a Lifetime Achievement Award from the Fresno County Office of Education. Mayor Dyer has been married to his wife, Diane, for 43 years and has been blessed with two children and five grandchildren. He and his wife attend New Covenant Community Church in Fresno.
