Wow, your amazing volcanic activity reminded me to request your island citizens a question. I would love to live on the Big Island as I know that you have horses!! I am in the process of 'Porting' to San Francisco, my hometown, in fact I was born in 1965 and that story is as that story goes however in inasmuch my mother and father decided to call it forward and made the lives of two others marvelous!! Now that I am 53 years old, single and my four kids are grown I believe that living where people are the happiest on earth would be fantastic. The fact that those plate lunches have anything to do with this would only admit that I loved those stands!! Summary: L4-5 herniation after a work injury 1999. In 2000 I had an L4-5 laminectomy and discectomy and subsequently returned to work as a supervisor for PetSMart. In December of 2004, I reinjured my back while picking-up a paper towel (as required by the District Manager or consequence of Store to impact and Quarterly Report would be severely impacted) in the Ladies Room at PetSmart Store #0053. Clinical Case Summary; dated 08/09/2018: The injured worker was a 52 year old, who sustained an industrial injury on July 27, 1999. The injured worker was undergoing treatment for radiculopathy of the lumbar region and L4-L5 anterior discectomy, decompression and fusion with instrumentation with an iliac rest bone graph.
According to progress note of May 11, 2017; the injured worker's complaint was low back pain. The pain was rated 10 out of 10 without medication and the average pain was 7 out of 10. The pain was rate 5-6 out of 10 with medication. The injured work no medications at this time. The injured worker was requesting medications for the rear times when the symptoms were intense to deal with utilizing over the counter medications. The injured worker was only taking the over the counter medications at this time. The objective findings were a well-developed and well-nourished injured worker in no apparent distress. The injured worker walked in a mildly stooped fashion. The injured worker had a stooped gait. The lumbar spine examination noted no limitation in range of motion. The heel to toe ambulation was normal. The Babinski's test, straight leg raises and Pace's sings were all negative. The recommendation was a low dose Morphine to use on rare occasions seemed to be sufficient.
The injured worker previously received the following treatments Morphine Sulfate ER 10mg one tablet every 8 hours as needed since April 2017 and lumbar spine MRI on January 17, 2005. The RFA (request for authorization) dated June 19, 2017; the following prescription was requested for Morphine Sulfate ER 10mg#30. The UR (utilization review board) denied certification on June 22, 2017; prescription for Morphine Sulfate ER 10mg #30. Update: Prescription for Morphine Sulfate ER 10mg #30 is still held in pharmacy in The County of Marin awaiting approval from Arrowpoint Capital Worker's Compensation Insurance Company in-order for the Worker's Company to be charged and not requiring me to pay for the prescription and then seek reimbursement, notoriously done however due to the financial restrictions following the loss of income in 2007 I have had to now and currently wait for the Worker's Compensation Companies to pay in-full for every prescription. This process of approval extends opportunity to the Worker's Compensation Company to deny the request. A long and arduous process however this approach from Insurance Company both 'public and private' are not uncommon practices.

Clinical Case Summary; dated 08/09/2017: The injured worker was a 52 year old, who sustained an industrial injury on July 27, 1999. The injured worker was undergoing treatment for radiculopathy of the lumbar region and L4-L5 anterior discectomy, decompression and fusion with instrumentation with an iliac rest bone graph.
According to progress note of May 11, 2017; the injured worker’s complaint was low back pain. The pain was rated 10 out of 10 without medication and the average pain was 7 out of 10. The pain was rate 5-6 out of 10 with medication. The injured work no medications at this time. The injured worker was requesting medications for the rear times when the symptoms were intense to deal with utilizing over the counter medications. The injured worker was only taking the over the counter medications at this time. The objective findings were a well-developed and well-nourished injured worker in no apparent distress. The injured worker walked in a mildly stooped fashion. The injured worker had a stooped gait. The lumbar spine examination noted no limitation in range of motion. The heel to toe ambulation was normal. The Babinski’s test, straight leg raises and Pace’s sings were all negative. The recommendation was a low dose Morphine to use on rare occasions seemed to be sufficient.
The injured worker previously received the following treatments Morphine Sulfate ER 10mg one tablet every 8 hours as needed since April 2017 and lumbar spine MRI on January 17, 2005. The RFA (request for authorization) dated June 19, 2017; the following prescription was requested for Morphine Sulfate ER 10mg#30. The UR (utilization review board) denied certification on June 22, 2017; prescription for Morphine Sulfate ER 10mg #30. Update: Prescription for Morphine Sulfate ER 10mg #30 is still held in pharmacy in The County of Marin awaiting approval from Arrowpoint Capital Worker’s Compensation Insurance Company in-order for the Worker’s Company to be charged and not requiring me to pay for the prescription and then seek reimbursement, notoriously done however due to the financial restrictions following the loss of income in 2007 I have had to now and currently wait for the Worker’s Compensation Companies to pay in-full for every prescription. This process of approval extends opportunity to the Worker’s Compensation Company to deny the request. A long and arduous process however this approach from Insurance Company both ‘public and private’ are not uncommon practices.

Entire Agreement: This Agreement consists of the foregoing paragraphs not limited to but included with the Resume of Karen A. Placek to present. The intent for this document is for the consideration of ‘Residential Tenancy Request’ and as such the Introduction and Resume identified readily requires me to respectfully thank you for reading my request of tenancy and introduction to your rental property: 1 bedroom Unit, San Francisco, California. Tenancy and this Agreement shall not be construed against either Tenant or Owner(s), Agent(s), and/or, Landlord(s) on the ground(s) that such person authored or drafted this Agreement of ‘Request for Residential Tenancy’. The foregoing constitutes the entire Agreement of a ‘Request for Residential Tenancy’ between the parties. On-going; this Residential Tenancy Agreement between both parties is recognized now as a negotiating process. All parties agree that this Residential Tenancy Request serves as a ‘Letter of Introduction’ only and is in submission to further communication of interest in the property (1 bedroom, Unit in San Francisco, CA.) of the Owner(s), Agents(s), and/or Landlord(s).
Additional: Violation by Owner(s), Agent(s), and/or Landlord(s) of any applicable ordinance or statute shall be deemed sufficient cause to withdraw and terminate my interest and further communication in regards to your property (1 bedroom, Unit). Notwithstanding any other provision this Request for Residential Tenancy is communicated in good faith, each and every expressed part of this Letter of Introduction is to my knowledge true to form and considered public information (May 12, 2018: World Wide Web) and to be used at discretion of common sense.
This ‘Request for Residential Tenancy’ may be used for both rent-controlled and non-rent-controlled premises. If the premises being rented is not subject to any applicable rent control ordinance no preference in this Request for Residential Tenancy is subject thereto, nor shall such reference make any ordinance in any way applicable to the tenancy created by this Agreement of Introduction.
